Explain mobility, medicines, communication, meals, toileting and overnight needs privately to the service.
Confirm assessment requirements, availability, transport, contribution and cancellation terms. Do not assume approval guarantees a place.
Where possible, arrange an introductory visit or shorter trial. Record routines and a contact for concerns.
Agree the handover, collection time and what happens if the usual carer cannot resume care as planned.

Prepare a one-page handover

Include preferred name, important routines, allergies, current care instructions, contacts and equipment. Ask the provider how medicines and records should be transferred securely. Avoid relying on a handwritten medicines list without clinical confirmation.
