When could support start, and is there a waiting list?
Who will visit and how is continuity managed?
What happens if a worker is unavailable?
What are all the charges, including travel and cancellations?
How can the person raise a concern or change the arrangement?

Ask for a written service description and itemised quote. Record answers from each provider so you can compare like with like.
