Medicare vs Medicaid for elder care

Medicare and Medicaid are different programmes. Medicare generally does not cover long-term custodial care, such as ongoing help with dressing or bathing when that is the only care needed. Medicaid can support long-term services for eligible people, with rules and access that depend on the state and programme.
Why this distinction matters
A person may have health coverage but still need another way to pay for ongoing daily support. Before choosing a service, identify the exact service, the proposed payer and the written conditions of coverage.
Plan your next steps
Ask the right organisation
- Ask Medicare or your Medicare plan about the specific skilled service, setting, authorisation and out-of-pocket cost.
- Ask your state Medicaid agency about financial and care-needs eligibility for the relevant long-term-care programme.
- Ask whether a home and community-based option is available, whether there is a waiting list and which providers participate.
- If both programmes are involved, ask who coordinates the benefits and which organisation pays each part.
- Keep notices, dates, reference numbers and any appeal instructions. Ask for an explanation of a decision you do not understand.
Avoid assumptions
Do not assume a rehabilitation stay funds a permanent nursing-home placement, or that a provider accepting Medicare also accepts the relevant Medicaid programme. Check the exact service and payment arrangement. Do not transfer assets based on online eligibility tips; seek qualified advice about the consequences.

Can a person have both?
Some people qualify for both programmes. That does not mean every care service is covered. Ask for a benefits review focused on the care actually needed.
