Clarify the care needed

If nursing care is recommended, ask the treating professional to explain the tasks, frequency and expected duration in writing. Clarify which tasks need a qualified clinician and which can be supported by a care worker.

Keep the person involved in decisions and ask how the home-care team will communicate with their usual health professionals.

Is a referral or assessment needed?
Who is responsible for the clinical care plan?
How will changes or concerns be communicated?
Who supplies equipment and consumables?
What happens outside scheduled visit times?

Confirm arrangements

Request written visit details, costs and contact arrangements. Check coverage with the relevant programme or insurer rather than assuming all nursing visits are funded.
